Nakizito Kazigo, MD
A graduate of the United States Military Academy at West Point and Howard University’s medical school, Dr. Kazigo served in the U.S. Army as an engineer officer and physician. In 2006, she was deployed to Jalalabad, Afghanistan, at what is now called Forward Operating Base Fenty, in support of Operation Enduring Freedom. She deployed again in 2010 to Camp Kalsu in Iraq in support of Operation Iraqi Freedom.
Dr. Kazigo has extensive experience in managing the chronic conditions found in the geriatric population with an emphasis on treating repeated exacerbations of chronic disease in a nursing home setting.
She is a member of the American Medical Directors Association (AMDA) and serves as a medical director for multiple nursing facilities as well as hospice medical director for Hospice Care Partners.
When not working, Dr. Kazigo enjoys running, spending time with her family, and teaching basic math skills and first aid to schoolchildren and Scouts. She participates annually in the Army Ten-Miler and runs a small summer math group, Viridian Mathemagicians, alongside local volunteers.
Dano Jukanovich, Chief Financial Officer
Dano is a business builder, integrator and strategic thinker committed to execution. Serving in the dual COO/CFO role, Dano most recently helped lead the turnaround of a PE-backed $100M national healthcare diagnostics franchise system.
Prior to that, Dano built one of the leading social impact consulting and private equity firms in East Africa – advising CEOs and Heads of State and growing multiple businesses with clients like Kate Spade New York and L’Oreal.
He is a West Point graduate, Army Ranger, Ironman Triathlete, and has an MBA from Wharton and MA from Johns Hopkins SAIS. Earlier in his career, Dano was Director of Finance for AT&T Wireless and Philips Healthcare and spent five years as Co-owner and CEO of a mid-sized construction and real estate development company in the Pacific Northwest.
Dano has served on various non-profit Boards of Directors and remains an active advisor to social impact organizations in East Africa.
Jeff Perry, Chief Operating Officer
Jeff Perry is the Chief Operating Officer of LTHC Solutions and LTHCareline, which are focused on providing clinical services for skilled nursing and rehabilitation facilities in the Dallas – Fort Worth region of Texas and national telemedicine services for skilled nursing and rehabilitation facilities respectively.
A graduate of the United States Military Academy at West Point (BS) and Wilmington University (MBA), Jeff served in the U.S. Army as an aviation officer from 1995 to 2002.
Before joining LTHC Solutions, Jeff led global operations teams in eCommerce, cloud computing, and big box retail. His teams were as large at 3,700 members, spread across 13 countries, and supported customers in 57 countries. He launched and scaled support operations for new three eCommerce businesses at Amazon.com and one cloud computing service for Oracle. He also pioneered cross-functional programs yielding over $20M in annual cost savings at Amazon.com.
Jeff loves spending time with his beautiful wife and three amazing kids doing anything physically active. He is also a CrossFit geek, enjoys watching movies, and reading.
Annette Sanders, Practice Manager
After over 35 years as a Licensed Vocational Nurse, Annette Sanders joined LTHC Solutions as our Practice Manager. She earned her LVN license from Kiamichi Tech.
Before becoming a nurse, Annette proudly served in the US Navy and attributes her time in uniform to shaping her strong core values.
Much of her nursing career was serving the geriatric population as an Assistant Director of Nursing in skilled nursing facilities. Her extensive experience in this industry has given her keen insight into both the business and operational components of long-term care. Annette provides key resources, communication, training, and support to both the LTHC Solutions staff and our customers in addition to office management, overseeing claims processing, credentialing, and marketing.
Annette and her family reside in the DFW metroplex area where she enjoys spending time with family, writing poetry, music, drawing, and traveling – but her greatest passion is rescuing animals.
Johnita Darton, MD
Johnita Darton, MD, is a board-certified internal medicine physician and the medical director of Long-Term Health Care (LTHC) Solutions. A graduate of the University of North Carolina at Chapel Hill and East Carolina University’s medical school, Dr. Darton has worked as a hospitalist for more than 15 years managing chronic medical conditions affecting the geriatric population.
Most recently, she worked at Medical City Weatherford as a hospitalist managing acutely ill patients. She has also cared for patients at College Park and Hilltop nursing facilities in that community. Dr. Darton is an adjunct clinical assistant professor at University of North Texas and has been very active in teaching future doctors to care for an aging community.
She and her family have lived in Texas since 2012. Dr. Darton serves on the Aledo Education Foundation Board, a nonprofit organization that raises funds to help with innovative educational projects and activities that are beyond traditional funding in the Aledo ISD school district. In addition, Dr. Darton is a member of the Young Men’s Service Association, a mother-son organization that encourages high school boys to go out into their communities and volunteer their time at places such as Pythian Home, Tarrant Food Bank, and Justin’s Place, just to name a few.
Uroje Mirza, MD
Dr. Uroje Mirza is originally from Pakistan and is a board-certified Family Medicine physician. She joined LTHC Solutions in 2022.
She chose the field of Family Medicine because she believes it is the bedrock of medicine. While she sees patients of all ages, she is especially interested in Geriatric Medicine. She has over 5 years of experience treating the elderly population and her area of expertise includes but is not limited to management of chronic conditions such as Diabetes, Hypertension, Heart Disease, COPD, and preventative care. Her goal is to enable her patients to live their lives to the fullest by incorporating a balanced diet and active lifestyle.
In her spare time, Dr. Mirza enjoys spending time with her husband and 2 kids, fitness training, traveling, shopping, dancing, and binge-watching TV shows.
Lisa Baker, PA
Lisa attended the University of Texas at Arlington, Texas, graduating Magna Cum Laude with a Bachelor of Science degree in Biology. She attended the University of North Texas Health Science Center of Fort Worth, Texas, obtaining her master’s degree in physician assistant studies. She was an Adjunct Clinical Professor and Clinical Preceptor with the University of North Texas Health Science Center for over 10 years.
For 15 years prior to joining LTHC Solutions, Lisa coordinated and managed acute and chronic medical conditions for patients in long-term care, rehabilitation, and skilled nursing facilities.
She has been a member of several professional organizations including American Academy of Physician Associates and Texas Academy of Physician Assistants.
Lisa enjoys spending time with her husband, family and 4 dogs, square dancing, RVing, landscape and abstract painting, and reading.
Sherraine Edwards, NP
Scherraine attained her Bachelor of Nursing at Saint Joseph College in West Hartford Connecticut in 2009. Thereafter, she worked as a bedside nurse in long-term care, inpatient rehab, critical care, and medical-surgical units.
Scherraine went on to complete her Master of Science in Nursing with a focus as an adult-gerontology nurse practitioner from Maryville University in 2014. Since then, she has spent her career in the skilled and long-term care setting working with the geriatric population in providing medical management of acute and chronic conditions in addition to providing psychiatric care management.
Scherraine and her family live in North Dallas and in her spare time she enjoys resting and spending time with family and friends.
Caroline Elad, NP
Caroline Elad started her nursing career in 2005 and obtained her masters in nursing in 2020. Her journey has gained her experience in occupational health, urgent care, and post-acute. However, she is most passionate about adult gerontology, which led her to LTHC Solutions and a role as a dedicated telehealth clinician.
She is originally from Cameroon in Africa, where most of her family including her 90-year-old mother, still reside. Caroline and her husband, Frederick, lived in Kansas for 20 years, but they are quickly settling in their new home state, Texas. They have two grown sons.
For fun, she enjoys spending time with my family and friends and she loves to travel and cook.
Caroline is fluent in English and in French, her village dialect.
Shauntorya Jackson, NP
Shauntorya Jackson is a board-certified Adult-Gerontology nurse practitioner. She originally attended the University of Southern Mississippi, where she received a Bachelor of Science Degree in Nursing.
While working in various roles on the medical-surgical unit, she completed her Master of Science from the University of South Alabama. As a nurse practitioner she has worked primarily in skilled nursing, long-term care, and primary care settings managing chronic and acute medical conditions affecting the geriatric population. Her extensive experience was a welcome addition to the LTHC Solutions team when she joined as a dedicated telehealth clinician.
Shauntorya is originally from Mississippi, but she and her family settled in the DFW metroplex in 2011.
In her free time, Shauntorya enjoys spending time with her husband and three kids. She also takes pleasure in reading, shopping, and interior decorating.
Joyce Kenebrew, MS, RN, NP-C
Joyce attended Baylor University in Waco, Texas, as a pre-med student, but eventually switched to nursing and received her bachelor of science in nursing from Texas Woman’s University (TWU) in Houston. After briefly working as a registered nurse, she returned to TWU to obtain her master’s in nursing and became a certified family nurse practitioner in 2012.
Prior to joining LTHC, Joyce worked at MD Anderson Cancer Center as a nurse extern, registered nurse, then advanced practice nurse. In addition to her working in the field of oncology, Joyce has experience in pain management and helping patient manage chronic health conditions. She has worked in home health care and at a federally qualified health clinic that treated patients from 81 different countries.
Joyce is dedicated to caring for the geriatric population, managing acute care illnesses, and providing care to long-term residents at the skilled nursing facilities LTHC serves.
When not caring for patients, Joyce enjoys spending time with her husband and son. She enjoys cooking, baking, and playing sports. Joyce values her religious faith and considers her career in nursing a ministry.
Eunice Orji, MSN, FNP-BC
Eunice began her nursing career 20 years ago in hospitals focusing on Med Surg, Pre-Op, Rehab, and psychiatric nursing. She served as the Charge for a large Rehab unit. Eunice is currently board certified as a Family Nurse Practitioner. She completed her Bachelor and Master of Science in nursing at the University of Arkansas for Medical Sciences in 2002 and 2011 with a graduate program focus as a Family Nurse Practitioner.
She has been a Family Nurse Practitioner for 10 years and has an extensive background in taking care of geriatric patients. She has worked in numerous long-term care facilities in the DFW area. Eunice is an active member of several professional organizations including Texas Nurse Practitioners and Sigma Theta Tau, the International Honor Society of Nursing.
Eunice and her family reside in the DFW metroplex. She enjoys running, shopping, and reading.
Jackie Thomas, MSN, FNP-C
Jackie Thomas started her nursing career as a Paramedic in the US Army. After proudly serving her country, she continued her education to become a Registered Nurse and worked in acute care, long-term care, emergency rooms and additional specialties for over 13 years.
She chose to continue her education and earned her master’s degree in nursing from Herzing University, with a specialty in Family Health. Jackie joined LTHC Solutions as a Nurse Practitioner in 2023 and was one of our first dedicated telehealth clinicians. She strives to ensure all our patients receive the special care they deserve so they can enjoy the best quality of life possible.
When Jackie is not providing outstanding care to patients, she loves to spend time with her family and friends and travel.
Denaasia Bell, Data Entry Clerk
Denaasia has over 5 years of experience in the data entry and administrative field. She joined LTHC Solutions in 2023 as a Data Entry Clerk and plays a vital role in our revenue management cycle. She prepares, gathers, and organizes crucial information for swift and accurate filing and reporting.
Prior to joining the team Denaasia served in the United States Army as an Administrative Specialist. There she was responsible for a variety of clerical duties such as recording, storing, and delivering data to keep operations running smoothly. She is a graduate of San Antonio College, in San Antonio, TX, where she received an Associates in Kinesiology.
In her free time, Denaasia enjoys writing poetry, reading books, and spending time with her dogs. She also attends poetry slams in the DFW area.
Karen Myllykangas, Medical Assistant
Karen J. Myllykangas is a medical assistant at Long-Term Health Care (LTHC) Solutions.
A graduate of Bemidji State University in Bemidji, Minnesota, Karen worked as a teacher for 10 years before joining LTHC in 2017. Karen’s expertise includes proficiency at networking and administration that allows LTHC providers to seamlessly liaison with the skilled nursing facilities they serve throughout the Dallas-Fort Worth area.
At LTHC, Karen adeptly manages the insurance, admissions, and custodial paperwork for the 500+ patients that LTHC providers care for. This includes patient scheduling and practice management for LTHC healthcare providers. She processes H&Ps, maintains communication between LTHC clinicians and facility staff, and produces teaching materials. She also assures that DONs, ADONs, and administrators are updated on changes affecting patient care.
When she’s not working, Karen loves playing with her nephew, walking, reading, creating greeting cards, and playing games.
